to mind when we think about childhood. Most of us have a lovely picture of what life is like for children. What could children possibly have to worry about? In the early l980's, many medical experts believed that children were unable to experience depression (忧郁). But the fact is that children do have plenty of cares. At least 2% of children under 12 struggle with depression and by teenage years this has risen to 5% or 1 in 20. That's at least one depressed child in every classroom.

Depression, whether in grown-ups or children, is poorly understood. Some children are more at risk, such as those who are ill or who come from a broken family. But there are also many simpler worries, such as exam fears. It's not always easy to notice depression in children, partly because they are not so able as adults to express their feelings and partly because they are more likely to act out their feelings with bodily actions. So while some children are clearly sad, quiet, and tearful, others may become troublesome, hurting or fighting with others.

If you are worried about your child, you should talk to your doctor and to your children's friends, too, to try to learn about their feelings. Medicine may be used, although less often than they are in grown-ups. But it's very important that efforts are also made to help children understand why they became depressed, how they can deal with the problems in their life, and how they can form. a more positive (正面的) opinion of their world. Feeling down? Got the blues? You're not alone. Everyone gets. sad (yes, everyone you've ever met). Some people have sad feelings just once in a while, and others may have sad feelings pretty often. More than half of teenagers go through a sad period at least once a month and plenty of younger kids do, too.

When you're in a sad mood, it may feel like it will last forever, but usually feelings of sadness don't last very long a few hours, or maybe a day or two. A deeper, more intense kind of sadness that lasts a lot longer is called depression. Each year millions of people are estimated to suffer from depression, a condition so widespread that it has been dubbed "the common cold of mental illness. "

Symptoms of Depression

Depression is associated with many symptoms and not everyone has the same ones. Some people have many symptoms, while others may only have a few. The symptoms below may signal that you or someone you love may be depressed:

Appearance—sad face, slow movements, absent-minded look

Unhappy feelings—feeling sad, hopeless, discouraged, or listless

Negative thoughts—"I'm a failure," "I'm no good," "No one cares about me. "

Reduced activity—"I just sit around and mope," "Doing anything is just too much of an effort."

Reduced concentration.

People problems—"I don't want anybody to see me," "I feel so lonely."

Guilt and low self-esteem—"It's all my fault," "I should be punished."

Physical problems—Sleeping problems, weight loss or gain, decreased sexual interest, or headaches.

Suicidal thoughts or wishes—"I'd be better off dead," "I wonder if it hurts to die."

Causes of Depression

The exact cause of depression remains unclear. The most probable explanation is that it is an imbalance in neurotransmitters (神经传递素) in the brain. But what triggers it in the first place?

First, genetic factors. There is growing interest in genetic approaches t6 understanding a variety of diseases and depression is no exception. Some people certainly seem to be more vulnerable to depression than others and there is growing evidence that some genes could make individuals susceptible to the disease. Some studies have shown that people who have relatives with depression have a one in four chance of developing it themselves, compared with only one in fourteen for the general population.

Secondly, external triggers. In most cases just having these genes is probably not enough to cause depression on its own. Depression can be triggered by a number of factors such as unemployment, bereavement (丧亡), social isolation or even a severe physical illness.

Victims of Depression

Anyone may suffer from depression, regardless of age, gender, race and belief. It is "normal" for certain people to feel depressed-such as women, older people, employees with work-related stress, or people with chronic illness.

Women are more than twice as' likely as men to experience depression. Although the reasons for this difference are not known, research suggests that biological differences in women such as hormonal(荷尔蒙) changes and genetics-may contribute to depression. Additionally, social reasons, such as greater stresses from work and family responsibilities, and even the increased rates of sexual abuse and poverty among women, may lead to higher rates of clinical depression.

Depression affects employees at all levels of the corporate ladder. One in every 20 employees experiences the illness. An estimated 200 million workdays are lost each year due to employee depression. Depression tends to affect people in their prime working years and if left untreated may last a lifetime.

A Crisis of Confidence for Masters of the Universe

Meltdown. Collapse. Depression. Panic, The words would seem to apply equally to the global financial crisis and the effect of that crisis on the human psyche (精神). Of course, it is too soon to judge the tree psychiatric(精神上的) consequences of the economic breakdown; it will be some time before epidemiologists(流行病专家) can tell us for certain whether depression and suicide are on the rise. But there's no question that the crisis is leaving its mark on individuals, especially men.

One patient, a fund analyst, came to me recently in a state of great anxiety. "It's bad, but it might get a lot worse," I recall him saying. The anxiety was expected and appropriate: he had lost a great deal of his (and others') assets, and like the rest of us he had no idea where the bottom was. I would have been worried if he hadn't been anxious. Over the course of several weeks, with the help of some anti-anxiety medication, his panic subsided as he realized that he would most likely survive economically.

But then something else emerged. He came in one (lay looking subdued and plopped(扑通一声落下 ) down in the chair. "I'm over the anxiety, but now I feel that I'm a loser," He was not clinically depressed: his sleep, appetite and ability to enjoy himself outside of work were unchanged. This was different.

The problem was that his sense of success and accomplishment was intimately tied to his financial status; he did not know how to feel competent or good about himself without this external measure of his value.

He wasn't the only one. Over the last few months, I have seen a group of patients, all men, who experienced a near collapse in their self-esteem, though none of them were clinically depressed. Another patient summed it. up: "I used to be a master-of-the-universe kind of guy, but this cut me down to size."

I have plenty of female patients who work in finance at high levels, but none of them has had this kind of psychological reaction. I can't pretend this is a scientific survey, but I wonder if men are more likely than women 1o respond lifts way, At the risk of trading in gender stereotypes, do men rely disproportionately more on their work for their self-esteem than women do? Or are they just more vulnerable to the inevitable narcissistic injury that comes with performing poorly or losing one's job?

A different patient was puzzled not by his anxiety about the market, but by his total lack of self-confidence. He had always had an easy intuitive feel for finance. But in the wake of the market collapse, he seriously questioned his knowledge and skill.

Each of these patients experienced a sudden loss of the sense of mastery in the face of the financial meltdown and could not judge their success or failure without the only standard they knew --a financial profit.

The challenge of maintaining one's self-esteem without recognition or reward is daunting. Chances are that if you are impervious to self-doubt and go on feeling good about yourself in the face of failure, you have either won the temperamental sweepstakes or you have a real problem tolerating bad news.

Of course, the relationship between self-esteem and achievement can be circular. Some argue that the best way to build self-esteem is to tell people at every turn how nice, smart and talented they are. That is probably a bad idea if you think that self-esteem and recognition should be the result of accomplishment; you feel good about yourself, in part, because you have done something well. On the other hand, it is hard to imagine people taking the first step without first having some basic notion of self-confidence.

The waiting room for Hilarie Cash's practice has the look and feel of many a therapist's office, with comforting classical music, paintings of gentle swans and colorful flowers and on the bookshelves piles of brochures on how to get help.

But along with her patients, Dr. Cash, who runs Internet/Computer Addiction Services here in the city that is home to Microsoft, is a pioneer in a growing trend in mental health care and addiction(上瘾) recovery. The patients, including Mike, 34, are what Dr. Cash and other mental health professionals call onlineaholics. They even have a diagnosis(诊断结果): Internet addiction disorder.

These specialists estimate that 6 percent to 10 percent of the around 189 million Internet users in this country have a dependency that can be as destructive as alcoholism and drug addiction, and they are rushing to treat it. Most of such users are online to further addictions to gambling or pornography(色情资料). But other users have a broader dependency and spend hours online each day, surfing the Web, trading stocks or instant messaging and a fast-rising number are becoming addicted to Internet video games.

Dr. Cash and other professionals say that people who abuse the Internet are typically struggling with other problems, like depression and anxiety. But, they say, the Internet's offer of escape from reality, and the opportunity for keeping unknown can also attract otherwise healthy people into an addiction.

Dr. Cash and other therapists say they are seeing a growing number of teenagers and young adults as patients, who grew up spending hours on the computer, playing games and sending instant messages. These patients appear to have significant developmental problems, including attention disorder and a lack of social skills.

A report released during the summer by the Pew Internet and American Life Project found that teenagers did spend an increasing amount of time online: 51 percent of teenage Internet users are online daily, up from 42 percent in 2000. But the report did not find a fading of social skills. Most teenagers "maintain energetic networks of friends", it noted.

A growing number of therapists and recovery centers are often treating Web addicts with the same approaches used to treat chemical addictions. There is one inpatient program, at Proctor Hospital in Peoria, Illinois, which accepts patients to recover from online addiction. Experts there said they see similar signs in those patients as in alcoholics or drug addicts, including sweating, severe anxiety, etc. The hospital's treatment program places all its clients together for group therapy and other recovery work, whether the addiction is to drugs or the computer.

Several counselors and other experts said time spent on the computer was not important in diagnosing an addiction to the Internet. The question, they say, is whether Internet use is causing serious problems, including the loss of a job, marital difficulties, depression, isolation and anxiety, and still the user cannot stop. "The line is drawn with Internet addiction," said Mr. Zehr of Proctor Hospital, "When I'm no longer controlling my Internet use, it's controlling me".

Some therapists and Internet addiction treatment centers offer online counseling, including at least one 12-step program for video game addicts, which is controversial. Critics say that although it may be a way to catch the attention of someone who needs face-to-face treatment, it is more like to treat an alcoholic in a wine factory, mostly because Internet addicts need to break the cycle of living in cyberspace.
